Physician support
Coram’s mission is to be the provider of choice for all your home PN needs by giving you and your patients a level of service, responsiveness and care that is second to none. We provide initial and ongoing clinical support for each patient by assigning a specialized Home Nutrition Support Team comprised of an advanced practice nutrition support registered dietitian, a registered nurse and a pharmacist. Consultation with our Nutrition Medical Director is also available for complex cases.
Process for home initiation of PN
Our home initiation program includes patient identification and assessment for appropriateness of care, clinically validated home initiation guidelines, in-home training by a registered nurse, comprehensive nutrition assessments by registered dietitians and individual PN formula recommendations. Our goal is to safely and effectively initiate PN in the home while ensuring that you and your patients are fully supported throughout the process.
