Using EHR interoperability to deliver a simplified experience

Provider showing information on tablet with patient

Working with Coram®* just got easier. As a CVS Health® company, Coram is the only national home infusion provider on Epic®. We have secure access to patient clinical data within your EHR through the Carequality® interoperability network**. Now you can send infusion referrals directly through your EHR and reduce paperwork, coordinate transitions of care and help improve patient safety.

Providers can:

  • Get visibility to infusion nursing visits and reduce paperwork and calls
  • Monitor patient's response to therapy with bi-directional communication
  • Use Epic to send infusion referrals electronically and expedite PA support

Patients can:

  • Improve safety with smart therapy guides, decision support tools and medication barcode scanning
  • Access after-visit summaries, medication lists and treatment information all in one place with CVS Health MyChart®
  • Get personalized infusion support with 24/7 on-call clinical assistance

Our integrated approach

EHR interoperability enables secure access and transfer of essential health data.

  • Coram can access admission/discharge summaries, H&P and demographics, lab and diagnostic results and office visit notes
  • Providers have visibility to home infusion nursing visits, progress notes, clinical summaries, lab results and medications
  • Patients can conveniently use hospital or provider's patient portal to view their medical records and access CVS Health MyChart to view instructions and ask questions
